NHS Diabetes Prevention Programme
Every two minutes someone finds out that they have Type 2 diabetes, a potentially serious health condition that can cause long-term health problems. It can lead to sight loss, kidney failure, loss of a limb, and makes you at least twice as likely to have a heart attack or stroke.
Healthier You: NHS Diabetes Prevention Programme is part of the national programme which provides support to thousands of individuals each year, who are at risk of developing Type 2 diabetes.
Those referred to the service, provided by ICS Health & Wellbeing, receive tailored, personalised support to reduce their risk of Type 2 diabetes including education on healthy eating and lifestyle, help to lose weight and physical exercise programmes, all of which together have been proven to reduce the risk of developing the disease.
It can be tough to make lifestyle changes but with the right help it is possible to make simple changes that reduce the risk of developing Type 2 diabetes. ICS Health & Wellbeing help to put the health back in the hands of service users and reduce their risk of developing the condition. Previously, individuals have only been able to join the programme if they have a blood glucose reading from their healthcare professional that identifies them as at risk; however it is now possible to check your risk using the online tool, and self-refer onto the programme.
